DESCRIPTION OF WORK

Responsibilities of this position include the following:

  • Obtain social information for consumers for the development of treatment plans, and enrollment into Behavioral Health & Intellectual/Developmental Services under the supervision of the Supports Coordinator Supervisor.
  • Assist consumers in filing applications for benefits.
  • Participate in the development of Individual Service Plans for each consumer. Monitor Residential and Day Program providers to assure implementation of stated Individual Program Plan services.
  • Maintain files, records and other documentation necessary for the continuity of consumer care and development of life management plans, and act as an advocate for the consumer to ensure the most appropriate treatment is approved.
  • Coordinate and direct meetings to assess the consumers' progress in charted areas.
  • Access supportive services for the IDS clients residing in the community, including budgeting, shopping, leisure time activities, social skills, personal hygiene, and individual counseling in making adjustment to community life.
  • Attend resident reviews at State Centers, Residential Programs, Vocational Rehabilitation Facilities and other service providers to ensure adequate levels of service and implementation of programs designed for each client.
  • Provide crisis intervention and emergency services as coordinated by the supervisor of said services.
  • Provide technical assistance to service providers in maintaining compliance with 2176 Community Waiver Regulations and Person Family Direct Supports Waiver.
  • Perform other duties as assigned.

Work Schedule and Additional Information:
  • Full-time employment.
  • Work hours are 8:30 am to 4:30 pm, Monday - Friday, with a 30-minute lunch.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS
Minimum Experience and Training Requirements:

  • Six months of experience as a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • A bachelor's degree with a social welfare major; or
  • A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Other Requirements:
  • PA residency requirement is currently waived for this title.
  • You must be able to perform essential job functions.

Legal Requirements:
  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
