Overview

Job Profile Summary The Director of Value Based Care Performance is responsible for the performance in the defined service
area for value-based care contracts, overseeing hospital and physician/practice financial, utilization, and
quality performance for value-based care global financial risk, Clinically Integrated Network (CIN), and
shared shavings arrangements. This position, in partnership with the analytics team, identifies and
recommends various financial, utilization, and quality improvement initiatives and brings the appropriate
work teams together to execute on implementation plans and tactics. The Director provides outstanding
customer focused service and is the key liaison for the service area leadership on value-based care
including hospital and medical group operations leadership and aligned Clinically Integrated Network
physician practices.
MAJOR RESPONSIBILTIES
In a dyad relationship with service area medical leadership, responsible for the service area
action plans to identify and drive improvement in financial, utilization, and quality
performance.

Executes on Enterprise Population Health strategic value-based care initiatives to drive
improvement at the hospital and practice sites by bringing the appropriate work teams
together to implement necessary tactics.

Serves as the principal relationship manager, monitoring, maintaining, and continually
improving the relationship with service area hospital sites, ambulatory locations, and
physicians.

* Service area subject matter expert/content expert in value-based care.
Coordinate and participate in improvement collaboratives to identify best practice playbooks
for value-based care.

Responsible for the service area Governance and Value Based Care Core Team and
implantation of needed work teams.

Develop corrective action plans, if necessary, in partnership with the service area medical
leadership to be reviewed and approved by service area governance.

Serves as a resource to link for physicians and practices to the necessary managed care
operations, including provider relations, claims processing and referrals to ensure proper
handoffs and excellent customer service.

Performs human resources responsibilities for staff which include interviewing and selection
of new employees, promotions, staff development, performance evaluations, compensation
changes, resolution of employee concerns, corrective actions, terminations, and overall
employee morale.

Recommends operating budgets, service area global financial risk budgets, and controls
expenditures within approved budget objectives.

Responsible for understanding and adhering to the organization's Code of Ethical Conduct
and for ensuring that personal actions, and the actions of employees supervised, comply
with the policies, regulations, and laws applicable to the organization's business.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED
License/Registration/Certification Required: None
Education Required: Master's degree in Health Care or related field.
Experience Required: 7 years with a physician organization or managed care
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
Demonstrated ability to work and function in a complex corporate system environment with matrixed
reporting relationships.
Excellent written and verbal communication skills and the ability to communicate effectively with all levels of
employees in order to collaborate with staff/management/executives/other organizations.
Skills in designing, administering, and evaluating programs and practices to maximize organizational
performance.
Proven ability to develop sound proposals and successfully implement program/project plans.

Excellent analytical, quantitative, and qualitative skills to facilitate effective decision-making.
Ability to identify and solve problems creatively, to work within deadlines with a high attention to detail, and
to work independently in a fast paced, dynamic environment
