Job Summary

The Licensed Clinical Social Worker (LCSW) is a vital member of the Care Transitions Outreach Program (CTOP) with accountability for coverage in the Quality Incentive Program (QIP) working with Behavioral Health and Substance Abuse patients in the Office of Population Health. Responsibilities include using integrated technology in placing calls to patients after discharge to address and resolve issues, needs, and/or questions; and performing rapid service recovery for patient satisfaction issues utilizing the Living Pride philosophy in all actions dealing with internal and external customers. The LCSW works closely and collaboratively with other members of the program, inpatient/outpatient providers, and other members of the health care team through effective organization, communication, and documentation.

The LCSW provides psychosocial assessment, crisis intervention, brief therapeutic interventions, consultation, education, community linkages and supportive services for identified patients and families. The LCSW coordinates resources and services in response to patient's psychosocial needs. The LCSW has advanced knowledge and skills in Clinical Social Work and is distinguished from the MSW by leadership activities within the Care Transitions Team, Behavioral Health/Substance Abuse QIP Team, and Department of Social Work, including participation in advanced program planning, provision of educational programs and consultation to the clinical team, service lines, other departments, and the community. The LCSW demonstrates the ability to work with any patient regardless of race, age, gender, religious affiliation, cultural beliefs, lifestyle, disease process, or treatment; the LCSW plans for and monitors services and interventions ensuring provision of quality care.

This LCSW role is part of a dynamic Care team within the Office of Population Health (OPH) Clinical Programs. This position reports to the Manager of Care Transitions with oversight by the Manager of Behavioral Health. The LCSW is a fully remote position with some requirement for travel to San Francisco or the Brisbane Office to meet Divisional priorities. The incumbent should be able to provide a safe and private home office environment for conducting patient outreach and UCSF business.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $41.79 - $72.17 (Hourly Rate).

Department Description

The Office of Population Health (OPH) leads UCSF's development of innovative care models to improve the value and quality of care and improve health outcomes for identified populations and facilitates implementation of these care models across the health system. OPH is responsible for fulfilling UCSF Health's accountable care contracts and fostering care partnerships across a clinical network. This department is comprised of four main teams: (1) Clinical Programs, (2) Population Health Quality and Performance Improvement, 3) Ambulatory Risk Capture/Medicare Advantage Operations, and 4) Population Health Administration.

Required Qualifications

* Master's degree from a CSWE accredited school of Social Work.
* Licensed in Clinical Social Work in the state of California.
* Required three (3) years of social work experience
* Familiarity with an electronic medical record program and/or Microsoft Office, Excel, Word and Outlook
* Knowledge and abilities essential to the successful performance of the duties assigned to the position.
* Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.
* Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high volume work environment.
* Ability to work effectively and collaboratively with interdisciplinary teams.
* Demonstrates case management skills and experience in the community health care delivery system.
* Exceptional communication and presentation skills relating to all vulnerable and at-risk populations, their support systems and teams of health professionals.

Preferred Qualifications

* Bilingual fluency highly preferred: English plus one other language, particularly Cantonese speaking. Also consider Spanish, Russian, Tagalog, or Mandarin
* Two or more year experience in psychotherapy and Cognitive Behavioral Therapy

License/Certification

* Licensed in Clinical Social Work in the state of California.
