SUMMARY

We are currently seeking a Population Health Project Associate to join our Population Health team. This position is full-time with benefits. This role will primarily work hybrid (days per week is dependent on the need related to projects, meetings and onsite events) Monday - Friday, day shift.

Population Health Project Associate is an entry level role on the Population Health team. The Project Associate helps support a variety of care improvement projects working across Hennepin Healthcare. Primary focus is working with others to create and execute central outreach campaigns to patients with preventive care gaps (like cancer screening and immunizations). Tasks may include planning and documenting campaign efforts, data tracking and reporting, and individual patient follow up as needed. This position is full time and hybrid-working both from home as well as on campus in the hospital or clinics as needed.

This position will complete assigned project duties by working with HHS team members, patients, and community partners where appropriate. Will help create an understanding of assigned project with internal/external partners for ongoing support, collaboration, and adaptation in rapidly changing healthcare environments. The Project Associate is responsible for coordinating and implementing all tasks that relate to a specific project.

RESPONSIBILITIES



  1. Collaborates with administrative/departmental team to determine needs for assigned project(s)
  2. Partners with internal and external resources and conducts research and/or analysis to carry out assigned projects
  3. Presents findings and recommendations to appropriate audience
  4. Attends meetings and learns about HHS-wide initiatives and current activities relevant to assigned projects
  5. Assists with any other projects or activities within the department as needed
  6. Collaborates with team on project iterations based on findings
  7. Facilitates communication among patients, family members, and medical staff
  8. Performs all duties to support assigned project in support of patient's needs

QUALIFICATIONS

Minimum Education/ Work Experience:



  • Two (2) years of post-secondary education
  • Two (2) years' experience in ambulatory care or physician group practice


OR



  • An approved equivalent combination of education and experience.


Preferred Qualifications:



  • Bilingual candidates strongly encouraged to apply. Degree in field appropriate to position.

  • Strong PC skills, including MS Office (Word, Excel, Power Point) and ability to quickly learn new systems
