SUMMARY: We are currently seeking a Population Health Manager to join our Population Health team. This position is full-time with benefits. This role will primarily work hybrid (days per week is dependent on the need related to projects, meetings and onsite events) Monday - Friday, day shift. Purpose of this position: This role's primary function is to provide direction and leadership for population health tactics and programs which can expand operational capability to manage the health of populations. The role will lead a portfolio of Population Health projects and supervise a team of Population Health staff. The Manager may also establish and manage population health infrastructure programs and services. The Population Health Manager plays a key role on the Population Health team and helps set strategic direction for the department. This position reports to the Director of Population Health and partners closely with the Senior Medical Director, Population Health, along with other clinical and administrative leaders RESPONSIBILITIES: Population Health Project Execution:
- Develop and execute an annual work plan including goals, performance metrics and milestones for Population Health projects
- Partner with operations and clinical leadership to design and execute large-scale, multi-site projects that build foundational capabilities to manage the health of populations
- Partner closely with wide range of support stakeholders - IS&T, Quality, Performance Improvement - to plan and execute projects
- Employ a range of improvement methodologies to help advance goals
- Ensure evidence-based best practices, clinical guidelines, and data guide the project design and decision-making process
- Collaborate with HHS Epic partners to ensure supportive EMR tools, best practices, and reports are matched to care model change needs
- Create bi-directional communication and spaces for collaboration and co-creation with stakeholders
- Work in partnership with other strategic initiatives across the organization to align program goals and outcomes
- In partnership with clinic operations, lead care model or process changes into standard work protocols and performance expectations
Population Health Infrastructure:
- Leverage best practices in alignment with primary care and ambulatory principles such as team-based care
- Develop and manage population health infrastructure capabilities in partnership with ambulatory operations and other stakeholders
- Develop and track KPIs
- Hire and supervise staff as needed
Population Health Staff Supervision and Development
- Supervise and mentor team of population health staff
- Provide work direction and assignments to team members, supporting them to successful execution of their accountabilities
- Responsible for hiring, onboarding, and performance appraisals
- Manage and coordinate the activities of employee assignments
- Provide ongoing training and development
- Monitor key performance indicators
- Manage cost center budget(s)
Population Health Team Leadership
- Serve as key member of Population Health Leadership team
- Partner with other staff and leaders in Population Health team to align work and identify opportunities for synergy
- Lead department meetings as needed
- Participate in department strategic planning
- Represent Population Health in organizational level committees and workgroups
QUALIFICATIONS: Minimum Qualifications:
- Master's degree in business or healthcare related field + Five (5) years of population health or managing other types of healthcare change initiatives

- Knowledge of population health principles and techniques normally acquired through completion of a master's degree and deep experience + Ten (10) years of healthcare operations experience

- An approved equivalent combination of education and experience
Preferred Qualifications:
- Licensed healthcare practitioner
- 10 years of progressive experience leading/driving healthcare change
- 5+ years of experience in Ambulatory Operations
- Experience in the Hennepin Healthcare system
- Experience implementing Population Health capabilities in a health system
- Experience designing and facilitating large-scale group design and stakeholder sessions
- Experience in process/performance improvement
- Change management certification (e.g., Prosci)
Knowledge/ Skills/ Abilities:
- Knowledge of Population Health and/or value-based care
