Responsibilities
Provides symptom management of chronically/terminally ill patients within an interdisciplinary team environment. Conducts in home and telehealth assessments for patients with complex needs, including formulating a likely diagnosis, ordering and interpreting diagnostics and delivering appropriate care in line with patient's preferences and goals for care. - Prescribing new medications and performing medication reconciliation.
- Demonstrates sound clinical judgement and prescribing proficiency in all aspects of symptom management and end of life care.
- Able to facilitate family meetings, goals of care conversations and ACP discussions.
- Leading oversight of care plans in conjunction with the rest of the clinical care team.
- Advocating for the patient and caregiver in collaboration with other care providers to keep care as coordinated and consistent with patient's goals.
- Participate in weekly IDT meeting that may include physicians, nurses, medical social workers and community health workers.
- Communicates with primary and specialty providers, informing them of the plan of care and negotiating treatments.
- Performs all other duties as assigned.
Qualifications
Master's degree in nursing. Licensed as a nurse practitioner, specific to the state(s) you are assigned to work. Three years of experience in palliative care or geriatrics. - National certification in a primary field (adult, family, gerontology, acute adult, geriatrics).
