The Visiting Clinicians clinical manager is responsible for the day-to-day oversight of the home based CCM & PC program. The clinical manager is responsible for supervision, management, coordination and provision of quality patient care while demonstrating fiscal responsibility and maintaining the highest standards of care ensuring compliance with all policies, procedures and regulatory requirements. The clinical manager will have both direct and indirect clinical responsibilities over the team consisting of CHWs, RNs, an RN- team lead and MSWs. This role is accountable for measuring, monitoring and managing the achievement of key performance indicators and organizational clinical performance.
Responsibilities
- Demonstrates sound clinical judgement and prescribing proficiency in all aspects of symptom management and end of life care, excelling in the role as a nurse practitioner both clinically and as a leader.
- Facilitates family meetings, goals of care conversations, and ACP discussions.
- Participates in onboarding, orientation, and ongoing clinical education of the CCM and PC team.
- Promotes a team environment based on company mission, vision, and values.
- Acts as a clinical resource for the team and collaborates with, Director of Operations, and the lead physician of the local market on an ongoing basis.
- Exhibits a high degree of leadership expertise in problem solving, mission/practice standards, job performance skills, and communication.
- Collaborates with leadership and human resource recruiters to attract and retain CCM and PC staff and providers.
- Communicates organizational policies and procedures, evidence-based best practices, and regulatory/industry changes to direct reports.
- Supports direct reports in their achievement of individual professional growth goals through coaching, mentoring, evaluation and collaborative performance improvement expectations
- Collaborates with other leaders and teams to foster clinical excellence, fiscal stewardship, employee development and retention, as well as service line growth.
- Performs direct patient care when necessary for excellent patient and business outcomes.
- Monitors and evaluates employee performance. Holds employees accountable for the attainment of defined goals and objectives. Executes effective performance redirection when needed.
- Ensures compliance with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
- Performs all other duties as assigned.
Qualifications
Required:
- Current, unencumbered license to practice as a Registered Nurse (RN) specific to the state the employee is assigned to work for the company.Licensed as a Registered Nurse
- Three (3+) years of clinical experience in palliative care and/or geriatrics as a registered nurse.
- Two (2+) years of leadership including team-building and influencing growth. Team Lead experience accepted as leadership and supervisory role.
- Experience working in an interdisciplinary team environment.
Preferred:
- Home care, hospice, or palliative experience.
- Advanced Care Planning experience.
- BCLS and/or ACLS certification.
