Job Summary

Provides outpatient HIV medical case management services and patient advocacy through education, counseling, advocacy, benefits assessment and enrollment, and coordination of services. Ensures patient access to essential medications, medical care, and support services. Provides services to people living with HIV/AIDS who are medically managed through the grant-funded Ryan White Program - Division of Infectious Diseases.

Essential Functions

  • Conducts outreach activities to re-engage people living with HIV into outpatient medical care and services, working outside the office to conduct home visits and other outreach.

  • Follows up with new patients who have missed appointment(s) to link them to care.

  • Uses innovative and strategic methods to locate patients who have fallen out of care. Interviews patients with a compassionate approach to ensure successful linkage and retention, establishes strong rapport, and seamlessly transitions patients to Specialized Medical Case Management (SMCM). Provides a wealth of knowledge to clients on the benefits of medical care and the beneficial services offered within the agency.

  • Works with the Outreach Program Coordinator, SMCMs, MCMs, and other members of the interdisciplinary team to ensure patients are retained into medical care. Participates in monthly Outreach Workforce Committee meetings and in training sessions.

  • Performs Needs-Based Assessments, uses the Return to Care Assessment, and completes Intake Assessments as needed.

  • Serves as part of the patient's support system as needed at the beginning of the SMCM intervention. Communicates to supervisor service improvement suggestions and monitors problems that may increase the patient's risk for falling out of care. Sets priorities and keeps supervisor informed of progress, time tables and outcomes. Participates in program evaluation.

  • Maintains accurate and current records in HMIS and Provide Enterprise database systems. Manages a cohort of clients and systems to discreetly contact clients. Manages daily reports to review Outreach progress. Maintains and researches client information and completes documentation in a professional and timely manner in accordance with regulations.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Bachelor's degree in Social Work or related field of study.

  • Experience - One (1) year experience in health or social services related area
