ASSISTANT DIRECTOR CARE MANAGEMENT

The Assistant Director of Care Management will report to the Senior Director of Operations, Acute Care. The AD provides oversight of the Clinical Transition Care Coordinators (Union Nurses), facilitates and ensures effective throughput, identifies barriers to discharges and reports variances and complex issues to senior leadership. The AD ensures that pertinent policies and procedures are followed with respect to all legal, regulatory, and accreditation standards.

Must have NY State RN License, BSN degree with a minimum of 5 years of clinical nursing experience.

PRI Certification required; CCM certification preferred.
