Golisano Children's Hospital is seeking an experienced Case Manager RN to join their Health Start Program. One (1) year of acute care nursing is required for this position and scope of duties is listed below. This RN Case Manager will be traveling to patients homes.

Responsible for the delivery of care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Assists with directing and coordinating care for clients based on established clinical nursing practice. Collaborates with other professional disciplines to ensure effective and efficient care delivery and the achievement of desired outcomes. Supports the organization vision and mission. Uses knowledge of clients age and cultural diversity into the provision of care. Contributes to the provision of quality care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in care.

Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Healthy Start Team, and are expected in this role. Duties will include covering team member PTO, requested office tasks, and traveling outside normal work area regions to meet the needs of our patient caseload and the needs of the organization.

Scope of Duties: Professional nursing service is provided by a registered nurse and includes:

1. Initial and ongoing comprehensive assessments of the clients needs, at appropriate time points

2. Initiating the plan of care and revising as necessary

3. Counseling the client and family in meeting goals.

4. Referral to other services as needed

5. Informing the physician and other staff of changes in the clients needs

6. Evaluating outcomes of care

Graduate of an accredited school of registered nursing

Strong interpersonal/communication skills and organized work habits required. Perspective employees must possess the ability to effectively negotiate computerized documentation.
