Hennepin Healthcare (HHS) has a 0.50 FTE (40 hour per two week pay period) opening for a MTM Clinical Ambulatory Pharmacist in our HHS MTM Pharmacy Department. This position will work day shift, no weekends. This position will support our Ambulatory Pharmacy program by serving in our Positive Care Clinic (serving people living with HIV) and/or other clinics within our MTM enterprise, dependent on departmental needs. HHS Ambulatory Pharmacy services are delivered on-site, either on our main campus or in a community care clinic. This pharmacist will be responsible for providing Comprehensive Medication Management (CMM) in all clinic settings. Services are provided in primary care and specialty care clinics. In this role, it is necessary to demonstrate sensitivity, knowledge, and understanding of diverse academic, socioeconomic, gender, cultural, and ethnic backgrounds.

Our Pharmacists are credentialed and privileged through the Credentialing department at HHS.

RESPONSIBILITIES



  • Provide Medication Therapy Management and/or disease state management to clinic patients

    • Critically evaluate a patient's medication profile to ensure that each medication is indicated, effective, safe, and that the patient is adherent with therapy


  • Completes timely documentation of activities within the electronic medical record as appropriate for specific areas of responsibility
  • Responsible for maintaining accurate and proper patient medication profiles, monitoring to ensure the proper choice of medication, dosage form, strength, length of therapy and interaction potential of the prescribed medication
  • Provide medication information to patients, nurses, physicians or other health care practitioners
  • Be able to critically evaluate and interpret medical information when providing specific information to physicians or providers
  • Performs all responsibilities/duties required by the pharmacy as defined by the scope of service particular to each clinic to ensure that the unique nature of the client is addressed
  • May precept Resident or PharmD student rotations
  • Maintain current knowledge of pharmacotherapy, pathophysiology, and procedures in area of practice
  • During patient encounter, responsible for interpretation of provider medication orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepancies
  • During patient encounter, based on an assessment of the patient's specific medical needs, making interventions with the medical staff and nursing staff to facilitate optimal drug therapy and facilitate the provision of other pharmacy services
  • Keeps up to date with competencies of both the pharmacy and those required in the clinic of practice
  • Completes timely documentation of activities within the electronic medical record as appropriate for specific area of responsibility per policy from the Office of Medical Staff. Documentation is to be completed within 5 business days of encounter and no later than 30 days following the encounter
  • May administer vaccinations, as applicable
  • May work under Collaborative Practice and protocols as authorized by providers

QUALIFICATIONS
Minimum Education/ Work Experience:



  • Graduation from college with a degree in Pharmacy


Preferred Qualifications:



  • Currently practicing as an MTM provider in the state of MN --or--
  • Completion of a residency program with emphasis on ambulatory care within the past two years --or--
  • BPS certification


Knowledge/ Skills/ Abilities:



  • Excellent problem-solving & communication skills (verbal & written)
  • Must be a self-starter, able to work under pressure and be flexible in setting priorities
  • Ability to do accurate, detailed analysis of complex information and issues is essential
  • Extremely strong customer service orientation
  • Effectively interacts with team members and other departments
  • Adaptable to changing priorities, tasks, and work schedules to meet customer service standards


License/Certifications:



  • Possession of a valid license to practice pharmacy in the State of Minnesota
  • MTM provider certification achieved through completion of a DHS approved Accreditation Council of Pharmacy Education (ACPE) program that has both didactic and clinical elements within the past two years of employment
