The ideal manager for this position will be a Registered Nurse with 5+ years of leadership experience who holds a BSN or ASN with added BS/BA in a health care. This position is primarily remote, but travel may be needed at MaineHealth locations though out the organization. This position is Monday - Friday, no weekends or on call required.

Summary:
The Population Health Manager is responsible for the operational oversight of the Population Health program at Maine Medical Partners.

Required Minimum Knowledge, Skills, and Abilities (KSAs):

  1. Education: Bachelor's degree in nursing or an Associate's degree in nursing plus a BS/BA in a health care related field; or a degree in Social Work.
  2. License/Certifications: Current, unencumbered Maine RN license OR Licensed by the Maine State Board of Social Work required.
  3. Experience: Prior population health experience preferred. Minimum of 3 years' ambulatory care experience required.
  4. Mastery of theories, principles and methodologies underlying population health and the patient centered medical home preferred.
  5. Strong background in health promotion, chronic condition management, care coordination, motivational interviewing, care/case management and care coordination preferred.
