DESCRIPTION OF WORK

As an Aging Care Manager 2, you will conduct home visits to initiate care plans for consumers, respond to questions, and advise consumers of available community resources. Duties include conducting

reassessment activities including adjusting care plans in accordance with consumer needs, adjusting cost share program calculations, and adjusting level of care when necessary in accordance with PA. Department of Aging regulations and Area Agency on Aging policy. If needed, you will conduct field visits in response to emergency situations, monitor services, and complete all necessary data entry.

Other duties to include but not limited to:

  • Assisting consumers in negotiating the Aging service system by contacting providers concerning service delivery issues or a need for change in service delivery
  • Acting as an advocate and service broker for consumers with formal service providers not under subcontract with the AAA
  • Assisting consumers with housing/environmental issues such as landlord tenant disputes, foreclosures, utility problems, utility shut-offs, pest control, animal control
  • Consulting as needed with the Community Health Nurse on medical and care need issues
  • Acting as back-up to other care manager 2's in the Care Management Unit
  • Handling after hours 'crisis situations on a voluntary and rotating basis

Work Schedule and Additional Information:

  • Full-time employment.
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Carbon, Columbia, Lackawanna, Luzerne, Monroe, Schuylkill, Sullivan, or Wyoming counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.
