The Community Health Improvement Specialist supports Maricopa County's Community Health Improvement Plan (CHIP), and its collective (HIPMC) made up of various community-based organizations. In this position, you will collaborate with a diverse group of community partners to help identify, develop, implement, and evaluate community health assessments and initiatives that will directly impact the health of residents in Maricopa County. The successful candidate will be well organized, adaptable to new situations, and exhibit professional communication skills when engaging with partners, agency leadership, and public speaking. We Require
One year of experience in coordinating and administering public programs and services, program development, community engagement, coalition building or related areas Bachelor's degree in public health, program evaluation, health care administration, social work, or a related field A combination of post-secondary education and/or job-related experience may substitute for the minimum qualifications on a year-for-year basis

Project management or grant management experience Experience using virtual project management tools Knowledge of public health frameworks for community planning, social determinants of health, health equity, and community health needs assessments and improvement plans
Job Contributions
Coordinate the development and execution of a community-driven Community Health Improvement Plan (CHIP) that highlights priority areas as identified through the Community Health Needs Assessment (CHNA/CHA) Build and maintain frequent interaction with community interest groups, academia, community-based organizations, and healthcare and public/private agencies by obtaining their active support and participation in the Community Health Needs Assessments and Community Health Improvement Plan, in addition to other community health initiatives that address health equity, social determinants of health, and underserved populations Plan, develop, implement, lead, and evaluate the work of the Health Improvement Partnership for Maricopa County (HIPMC) through community health needs assessments, quarterly meetings, monthly engagement activities, social network analyses, workgroup projects, and other community assessment opportunities Assist in the collection, analysis, and communication of data to determine priorities, identify target populations, and educate community partners on areas of need and opportunities for collaboration and improvement Identify and deploy organizational resources that support community health improvement related to the four leading causes of chronic disease death; produce quality reports and documentation for partners and residents of Maricopa County Actively seek out and identify opportunities to engage internal and external partners in problem-solving, training and development, program evaluation and continuous improvement Support Maricopa County Department of Public Health internal worksite wellness initiatives Work in collaboration with the communication department to develop training and informational materials to promote public understanding of HIPMC, the CHNA/CHA and the CHIP to increase active participation and understanding
Working Conditions
Work is primarily in an office setting with telework options Irregular work hours may be required such as evenings and weekends to meet our community partners and residents where they are Must be able to participate in emergency response training as scheduled and be available to respond as directed in the event of a public health emergency Requires consistent collaboration with internal programs and community partners A high level of responsiveness and accountability is expected through various forms of communication during the workday Due to the nature of this position non-traditional work hours will occur such as evening and weekend to meet our community partners and residents where they are
