Responsibilities

What You'll Do:

As a Social Worker at PrairieCare, you will play a crucial role in enhancing the well-being and functioning of our patients through a comprehensive range of services, including:

  • Assessment & Planning: Conduct thorough psychosocial assessments and suicide risk assessments, ensuring a deep understanding of each patient's unique needs.
  • Treatment Development: Develop and implement individualized treatment plans, complete discharge plans, and facilitate discharge meetings to ensure smooth transitions for patients.
  • Family Engagement: Communicate effectively with patients and their families, involving them in treatment plan decisions and fostering a collaborative approach to care.
  • Care Coordination: Coordinate care with outpatient providers upon admission and discharge, ensuring continuity of care and support.
  • Team Collaboration: Facilitate weekly treatment team meetings, contributing clinical insights based on your expertise in family systems and social work theory.
  • Crisis Management: React quickly and calmly to handle potentially emergency situations, making effective decisions under pressure.
  • Group Facilitation: Lead psychoeducational and therapeutic groups, providing valuable support and education to patients.

Qualifications

  • What You Bring:

    • A Master's Degree in Social Work is preferred; a Social Work license in the State of Minnesota in required.
    • Experience in a healthcare setting is preferred, with a strong understanding of social work practices and principles.
    • Familiarity with electronic health records and proficiency in Microsoft Office products, including Word and Outlook.
    • Exceptional communication skills, both written and verbal, with the ability to convey information clearly and effectively.
    • Strong organizational skills, with the ability to smoothly transition between diverse tasks and manage time efficiently.
    • A deep understanding of gender, cultural differences, and age-related considerations in mental health care.
    • The ability to remain calm and composed in high-stress situations, contributing positively to a team-oriented environment.

Pay range for this position is $54,000 to $75,000 depending on education and experience.
