Provides daily oversight and strategic direction for all Medicare/Medicaid clinical services. Responsible for insuring member access to high quality, cost-effective care in accordance with corporate requirements and federal/state regulatory guidelines. Oversight of all field assessment activities, timely completion of all mandatory UAS-NY assessments, ensuring quality of these assessments, documentation and communication of assessment findings and date entry of assessments into HCS (Health Commerce System) according to all rules and regulations put forth by the Department of Health. Responsible for policy development and strategic planning including staff recruitment, retention, development and training. Ensures quality and accuracy in correlation of the data between UAS-NY, Tasking Tool and Paraprofessional Plan of Care (POC), and timely communication of assessment information to the Care Management Department.

  • BSN from accredited NLN-Accredited School of Nursing
  • MSN preferred
  • Licensed with current Registration to practice as Registered Professional Nurse in NYS
  • Minimum of 3 years Managed Long Term Care and UAS-NY experience required
  • Minimum of 3 years experience superving other RN's
  • Community Health or related field and/or minimum of two years of progressive job related experience, including field visits and coordination, education and supervision
  • Strong interpersonal and organizational skills
  • Excellent critical and problem solving skills
  • Working knowledge of State and Federal regulations
  • Ability to identify potential problems, escalate and make recommendations for solutions
  • Excellent written and verbal communication skills
