Position Summary
The primary purpose of this position is to serve as an Outpatient Intake Coordinator for the UNC Horizons Program at Wake County. This role will include completing screenings, intakes, referrals, and care coordination for women and children enrolled in the outpatient perinatal program at the UNC Horizons Program at Wake County. This person will join the interdisciplinary/clinical team to discuss clinical cases and assist with developing treatment plans for individuals in the program. This position will also focus on advocacy and clinical outreach for pregnant and parenting patients affected by substance use, trauma , violence, incarceration, and serves as a liaison with various stakeholders, while maintaining multiple community partner relationships through outreach. Lastly, this individual will support the referral line and consult with the clinical team to address identified clinical needs for clients and ensure that individuals are connected to clinical services upon completion or discharge from the program.
Minimum Education and Experience Requirements
Bachelor's degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.
Required Qualifications, Competencies, and Experience
Bachelor's degree in a human services discipline (Human Development, Social Work, Psychology, Sociology, etc.), including eligibility of becoming a Qualified Professional status. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board
Preferred Qualifications, Competencies, and Experience
History, knowledge, and at least one year of exposure to social work or counseling experience; or experience with the substance abuse population, gender-specific treatment, child and family counseling, justice-involved population and a perinatal setting preferred. An ideal candidate will have exceptional organizational skills, time management skills, charting and documentation, and ability to address clinical needs in a timely manner.
In addition, an ideal candidate would have the following experience:
1. Knowledge of SUD in adults
2. Knowledge of record keeping and administrative duties
3. Skill in assessment, case management, advocacy, and individual and group therapy.
4. Knowledge of the needs and concerns of women, including some exposure to issues such as childhood and adult trauma, domestic violence, sexual assault, substance abuse., pregnancy/parenting, and legal and economic difficulties.
5. Ability to work with women from varying incomes, ages, ethnic/racial groups, lifestyles.
6. Knowledge of residential environments
7. Knowledge of supervisory role, especially with paraprofessionals.

Specialized knowledge of motivational interviewing, 12-step Philosophy, trauma therapies, and recovery principals.
