SUMMARY

This position is responsible for training and leading caregivers in providing personal assistance and daily care and services related to the residents in order to enhance the residents' independence and quality of life, in accordance with the company's mission and values.

ESSENTIAL FUNCTIONS

The duties listed below are essential functions of the position that must be performed to effectively perform the position in at least an acceptable manner as defined by Front Porch. In the future, other duties may be assigned and may also be considered essential functions of the position.

* Assist residents in their Activates of Daily Living (ADL) as specified in the resident's care plan. This includes, but is not limited to, eating, bathing, dressing toileting, transferring (walking), and continence.

* Respond to residents' needs promptly while maintaining residents' self-respect, dignity, safety, and confidentiality.

* Provide companionship and emotional support to residents.

* Provide guidance to Caregivers in the performance of their duties.

* Monitor and record vital signs of residents.

* Communicate any observed or suspected resident change of condition to a supervisor immediately.

* Encourage teamwork through cooperative interactions with co-workers and other departments.

* Administer medication to residents in accordance to Title 22 regulations of the California Administrative Code, as needed.

* Perform all other related duties as assigned.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position.

* Must be able to remain in a stationary position 20% of the time

* Must be able to frequently move about inside the facility to access file cabinets, office machinery, etc.

* Must be able to constantly move to position self in order to conduct essential job functions when working with residents and their specific therapy needs

* Must be able to constantly operate a computer and other office productivity equipment, such as a calculator, copy machine, and computer printer to perform the essential functions of the job

* Must frequently move items weighing up to 40 pounds across office or facility for various needs

* Must occasionally ascends/descends stairs to provide essential services

* Must have the ability to succinctly communicate information and ideas so others will understand accurately exchanged information

* Must have the ability to observe details at close range (within a few feet of the observer)

* Must consistently work in an office and assisted living facility type atmosphere
