Helmerich Women's Health Center, Kaiser Rehabilitation Center, the latest technology for the treatment of cancer, and the Oklahoma Heart Institute. Responsibilities Act as liaison between the patient and medical personnel Facilitate long term health and well-being through counseling, education, and brokering of needed community services and resources Develop and implement appropriate treatment plans for each patient Serve as an advocate for patients and their families in regard to decisions affecting plan of care Act as a resource for hospice and counseling services Provide appropriate end of life planning and care Ensure timely discharge planning Qualifications Job Requirements Master's of Social Work degree Preferred Job Requirements Previous hospital experience Experience in Acute Care
