Responsibilities
  • Pediatric Patient Care & Care Coordination - Conducts comprehensive nursing assessments, develops individualized care plans, and provides high-quality, evidence-based home health services to pediatric patients. Educates patients and caregivers on treatment, disease management, medications, and emergency preparedness while ensuring safe and effective use of medical equipment.
  • Interdisciplinary Collaboration & Communication - Works closely with physicians, therapists, home health aides, and other healthcare professionals to coordinate patient care, update treatment plans, and ensure optimal health outcomes. Regularly communicates with families, providing support and education to enhance patient well-being and independence.
  • Compliance & Quality Assurance - Maintains accurate and timely clinical documentation in accordance with home health regulations, accreditation standards, and agency policies. Participates in quality assurance initiatives, performance improvement activities, and ongoing professional development to uphold best practices in pediatric home health care.

Qualifications

Education:

  • Graduate of State Board approved program for Registered Nurses

Experience:

  • One year nursing experience caring for similar patient population as to be assigned.

License(s)/Certification(s):

  • Valid license as RN in state(s) where providing care.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse.
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
