CASE MANAGER - FAMILY STABILIZATION ADVOCATE We are seeking a Case Manager - Family Stabilization Advocate. The role of the Family Stabilization Advocate is to provide Stabilization Services to families that have exited shelter or families that has been moved by the Front Door. The Stabilization Advocate will provide intensive case management services to families for 3 years, which includes ongoing barrier reduction, communication with property owners to ensure compliance, as well as appropriate referrals to aid the family to remain housed when Stabilization ends. The Family Stabilization Advocate will be responsible for providing case management services to families, using a trauma informed approach that include, developing monthly Rehousing Plans, referrals to community resources, advocacy, assessments, and crisis intervention, with the goal of placing families into permanent housing. Responsibilities include:
- Complete intake and needs assessments with each family, which may involve a broad range of areas including housing, finance & budgeting, employment, education, food & nutrition, school & daycare, legal issues, health, parenting, and daily living skills.
- Interview and evaluate clients to formulate Individualized Service Plans based on Program requirements.
- Assist families in securing permanent housing by making full use of all available housing opportunities including the private rental market and long-term subsidies including section 8, state aided public housing, state aided subsidies, and income eligible project based subsidies, as well as the HomeBASE Household Assistance Benefit.
- Work closely with families to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process.
- Maintain compliance standards of EOHLC Shelter Scope of Services.
- Help clients build self-sufficiency plans.
- Assist clients to transition to self-advocacy roles.
- Perform home visits and transport clients.
- Provide emergency response management.
- Establish and monitor goals.
- Work collaboratively with interdisciplinary team of collateral providers; Maintain accurate case records.
Qualifications:
- High School Diploma or Equivalent required.
- Prior related experience required.
- Minimum of one year assessment, advocacy and case management work in a Human Service field required.
- Knowledge and experience working with low income, culturally diverse, and at-risk families and individuals.
- Demonstrated knowledge of community resources available to low-income individuals and families.
- Excellent customer service, organizational, oral and written communication skills.
- Ability to manage multiple priorities and work in a fast-paced environment.
- Proficient computer skills required in basic computer software and data collection systems.
- Excellent crisis management skills.
- Ability to maintain accurate records and confidentiality of information.
- Excellent attention to detail.
- Valid Driver's License
- Bilingual (English/Spanish, English/ Haitian-Creole, English/Khmer, English/Portuguese) preferred.
