UNM Medical Group, Inc. is hiring for a Counselor Social Worker 1 to join our School Based Health Clinic. This position is a 10-month (August 2025 - May 2026) 1.0 clinical FTE (5 days a week). We are looking to fill this role by August 2025. As part of our integrated services, this position will provide therapy services to students attending APS schools. Will work with a team that will include, case management, medical provider, medical assistants and psychiatry fellows. Experience with adolescent aged children will be a plus. Hours: Monday thru Friday 7:45am - 3:45pm

Summary:

Provides, oversees, and/or administers a wide range of psychosocial evaluation/assessment, crisis intervention, and/or case management services in a high-intensity clinical environment which requires a high degree of independent decision-making and program administration.

Minimum Job Requirements of a Counselor Social Worker 1:

Master's degree in Counseling, Social Work, or a related degree. No experience required. Licensed eligible Counselor or Social Worker in the State of New Mexico, or eligible for license in the State of New Mexico required. Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Verification of education and licensure will be required if selected for hire.

Duties and Responsibilities of a Counselor Social Worker 1:



  1. Facilitates client access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, and relevant mental health services; assists client to develop natural resources and make contact with social support networks.
  2. Provides information, education and coaching in life skills (e.g. wellness promotion, parenting skills, job-search strategies, social skill building, and brief interventions) for clients and their families in a compassionate, professional and culturally responsive manner. Will also provide health literacy training as well as disease management skills.
  3. Engages clients in a process of ongoing information gathering and decision making in order to empower the client to identify their own goals, strengths and challenges.
  4. Serves as liaison, coordinator and/or advocate on behalf and in collaboration with patients to effectively transition care to routine primary care.
  5. Provides brief intervention to clients and families as appropriate to the position; contribute to that development of treatment plans, discharge plans, and documentation for coordination of care, follow-up care programs; provides therapeutic crisis intervention and emergency services as required.
  6. Collects data about patients through interview, case history, evaluates data to identify causes of problems and to determine proper therapeutic care necessary to engage patient in primary care or referral to other specialists.
  7. Consults with team members as appropriate, huddle and as needed to facilitate coordination of care; refers clients to appropriate service agencies as required.
  8. Arranges for emergent, urgent or therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious disturbance problems.
  9. Refers clients to case managers and/or appropriate social service agencies for financial assistance and other required services; may visits patients and families in their homes to provide counseling and outreach services.
  10. Establishes and maintains case files, assessments, brief intervention information, referrals, and other related documents for the treatment of clients.
  11. Consults with other legal and treatment agencies and individuals in relation to patient/client records, rights, and responsibilities.
  12. Plans and facilitates community outreach and education activities, as appropriate to the position; may serve as a liaison and representative to community organizations and schools.
  13. As appropriate to the position, performs various administrative functions such as monitoring budgets and preparing reports and correspondence, assists case management and nursing in coordination of patient care visits. May participate in a variety of research projects to develop service plans and may propose changes to program policies and procedures.

About the School Based Health Center Program

The School-Based Health Center Program provides services in 6 Albuquerque Public Schools. SBHCs provide health care and other services through a multidisciplinary integrated team to adolescents on the campus of the school. This position requires strong teamwork with other SBHC team members including Medical Assistants, Behavioral Health Providers, and Case Managers. The position requires active engagement with the school community to coordinate the provision of care and to promote SBHC services.
