Summary
The Physician Assistant (PA) is a licensed provider. The PA is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation. The Diabetes Physician Assistant works in conjunction with members of the Brigham & Women's Hospital Department of Endocrinology, Diabetes & Hypertension to care primarily for patients in the hospital in a consultative capacity. The patient populations include adults presenting to the Emergency Department, and inpatient medical/surgical setting(s) with hyperglycemia. Surgical services consult the Diabetes Management Service (DMS) for patients with perioperative hyperglycemia. The DMS is a 24-7 consult service directed by a physician leader who oversees a team of consultants that includes physicians, nurse practitioners, and physician assistants. NATURE AND SCOPE OF PRACTICE: The Diabetes PA will work as part of an inpatient consultative team and will see consults on DMS. This role may also include some weekend coverage. Specific responsibilities include, but are not limited to: Respond to consult requests received either electronically or by personal page. DMS consults are distributed by a designated DMS team member every day who will assign new consults and follow-ups to team members. Complete consults in collaboration with team members to deliver optimal care in a timely manner. Bill for services rendered per department standards. Create both written and oral recommendations to primary services for insulin initiation, administration, and adjustment. Create both written and oral recommendations for appropriate laboratory testing and medication management per clinical standards and/or Department specific algorithms. Collaborate with interdisciplinary inpatient teams to develop care plans for safe transition to home and outpatient follow up after discharge. Work collaboratively with team members to provide direct care, counseling and diabetes teaching to patients as needed.
