Nurse (RN) - Gastroenterology- MGH - Hybrid

Recall Program Development and Maintenance:

• Develop and implement a tracking system for high-risk and genetic condition patients to ensure consistent follow-up based on current clinical guidelines.
• Regularly update the tracking system to reflect changes in guidelines or institutional protocols.
• Collaborate with the clinic's quality improvement team to evaluate and refine processes.

Referral Triage:

• In collaboration with the Access team will serves as the point person to ensure timeliness in managing procedure referrals to the Division of Gastroenterology.
• Works closely with the administrative assistants and the Administrative Director of Clinical Operations to triage outpatient endoscopy requests that are urgent or require clarification.
• Will work closely with procedure scheduling administration to facilitate timely scheduling of urgent patients.

Patient Care Coordination:

• Serves as a nurse consultant for the Division, assisting with the handling of requests for patients with complex care needs and clinical concerns. Develops and implements care strategies utilizing a multidisciplinary approach to ensure optimal outcomes.
• Monitor recall data ensuring patients complete recommended surveillance procedures, such as colonoscopies, endoscopies, or imaging based on current guidelines.
• Serve as the primary point of contact for open access patients regarding their care plans, procedures, and follow-up needs. Collaborates with endoscopists to make alterations to the pre-procedure plan based in the clinical needs of the patient.
• Research, identify, and maintain an updated database of community resources, support groups, and services pertinent to gastroenterology patients.
• Establish partnerships and relationships with community organizations, social services, and support groups to enhance the availability of resources.
• Coordinate communication between the clinic, external providers, and community agencies to ensure seamless transitions of care.

Education:

• Acts as a resource to colleagues and consumers
• Assists with, Develops, implements, and evaluates educational offerings based upon learning needs of patients, families, and staff.
• Supports, participates, and contributes to educational programs for staff in a variety of role groups.
• Participates in a bi-annual review of clinical accuracy of prep documents. Optimize prep instructions based on data and collective feedback.
• Provide education to patients and families regarding clinical pre-procedure issues.
• Outreach and education to recall patients who are overdue for procedure/ imaging to reinforce recommended guidelines, and the importance of follow-up care.

Quality Improvement:

• Works closely with GI Quality and Safety group
• Track outcomes and metrics for high-risk patient follow-ups and provide reports to clinic leadership.
• Stay updated on new research, guidelines, and innovations related to gastroenterological conditions to integrate best practices into patient care.

Clinical Coverage:

• Provides coverage for GI division triage nurses as directed by the nurse manager based on the needs of the Division
