Job Summary

Essential Functions

• Provides personal care to include, bathing, dressing, feeding, toileting, transfers, lifting and incontinent care, to the PACE participants in their homes.

• Provides care to participants at the PACE Center.

• Responsible to report any changes in medical or physical status to the Home Care Supervisor PACE or designee.

• Performs procedures/treatmentsunder supervision of HCS or RN.

• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

• Education - High School Diploma or equivalent

• Experience - One (1) year nurse tech hospital experience or skilled nursing facility. One (1) year of experience working with the frail or elderly.

Required Certifications, Registrations, Licenses

• SC CNA

• CPR

• Valid driver's license, an acceptable three-year motor vehicle records as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.

Knowledge, Skills and Abilities

• Basic computer skills

• Knowledge of office equipment (fax/copier)
