*Essential Functions (Key Roles & Responsibilities)
*Work with an interdisciplinary care team of Emergency Department and Addiction Medicine clinicians to assist with patients' transition to further addiction treatment after discharge; take direction from team medical and clinical leadership as appropriate to optimize patient care.
*Proactively monitor patients with SUD who enter the Emergency Department to make suggestions to the responding clinician about accessing SUD services like consultation from the Addiction Consult Team or referral to the onsite outpatient Bridge Clinic.
*Make referrals to the full continuum of addiction treatment services inside and outside the system, including but not limited to outpatient treatment, opioid treatment programs, ATS, and CSS.
*Maintain an up-to-date understanding of local SUD and related resources and best practices for referral.
*Provide assistance to individuals/families in accessing a variety of public services that may address barriers to patient follow-up with addiction treatment including financial and medical assistance and directly help to meet needs for food, shelter, transportation, and clothing.
*Complete follow up phone calls to patients to ensure a successful transition to the treatment organization in the disposition plan.
*Work as part of a Resource Specialist care team which may include cross coverage in other SUD care settings onsite like the inpatient Addiction Consult Team or the outpatient Bridge Clinic.
*Performs other duties as assigned
*Complies with all policies and standards

High School Diploma or Equivalent required or Associate's Degree Related Field of Study preferred

Licenses and Credentials

Experience
related experience in the Addiction field 2-3 years preferred

Knowledge, Skills and Abilities
- Familiarity with addiction treatment, systems of care, and community resources or a willingness to learn.
- Ability to maintain effective working relationships with patients/families.
- Knowledge of current community resources.
- Strong assessment and crisis intervention.
- Strong collaborative skills and a desire to work in a complex, fast-paced environment.
- Excellent interpersonal skills.
