Responsibilities

As our Continuing Care Planner, you'll be the ultimate connector and guide, serving as a liaison for case management and discharge planning within our facility. You'll work closely with our fantastic Clinical Outreach Team and various stakeholders to ensure our clients transition smoothly to their next steps. Your role will be essential in helping families navigate the continuing care process while keeping the energy high and the smiles bright!

What You'll Do:

  • Team Up: Collaborate with the Clinical Outreach Team to coordinate transitions in levels of care and release planning-let's make it a seamless adventure!
  • Family Champion: Meet with families during the treatment process to explain continuing care procedures and processes, ensuring they feel informed and empowered every step of the way.
  • Discharge Dynamo: Work with the clinical team, utilization review specialists, families, and outside consultants to manage discharge dates, level of care recommendations, and transition plans. You'll be the go-to expert for all things discharge!
  • Plan Creator: Initiate involvement with patients, families, and staff to develop comprehensive discharge plans and continuing care treatment strategies that truly make a difference.
  • Support Network Builder: Help patients and families identify their support network and provide guidance on future care, follow-up, and any concerns upon discharge. You're their biggest cheerleader!
  • Documentation Wizard: Assist patients and families in completing all necessary clinical documentation for a smooth transition to their next programs or social service organizations. Paperwork? No problem!
  • Placement Advisor: Meet with patients and families to discuss placement options, organize records for transition, and provide advice on community assistance organizations and transportation options. You've got the inside scoop!
  • Educator & Advocate: Coordinate and participate in educational and training activities related to continuing care planning for patients, including travel to visit programs and facilities both locally and nationally. Adventure awaits!

Qualifications

  • Minimum of one (1) experience in a clinical setting.
  • High school diploma or GED required.
  • Knowledge and experience in the development of mental health discharge plans and coordination with community service providers, preferred.
  • Bachelor's degree in psychology, counseling, or sociology preferred.
