City of Hope Orange County is transforming cancer treatment by bringing world-class expertise and specialized care closer to home. Supported by a regional network of care, our 11-acre academic cancer campus in Irvine is home to City of Hope Orange County Lennar Foundation Cancer Center, our outpatient cancer center, and Orange County's only cancer specialty hospital, opening in late 2025.

With the opening of the hospital, the full continuum of advanced cancer care-from prevention through survivorship-will be available in Orange County for the first time, eliminating the need to travel for lifesaving treatment. Seamlessly connected to our outpatient cancer center, the new clinical research hospital will feature 73 inpatient beds, advanced surgical suites, groundbreaking clinical trials, and holistic services such as rehabilitative therapy and spiritual care. This isn't standard cancer care. This is lifesaving cancer care. This is Hope.

The Clinical Pharmacist II operates at the highest level of clinical practice, collaborating as a key member of the multidisciplinary healthcare team to provide direct patient care. In the inpatient hospital setting, this pharmacist takes ownership of workflows and processes within their area of practice, contributing to the design, implementation, documentation, and monitoring of comprehensive medication management (CMM) and therapeutic care plans. Through direct interactions with patients and providers, they ensure optimal, patient-centered outcomes.
Serving as a vital link between Pharmacy Services and Hospital & Specialty Oncology Services, the inpatient clinical pharmacist plays a crucial role in facilitating discharges and coordinating care to enhance continuity and improve patient outcomes.

As a successful candidate, you will:

  • Provides comprehensive medication management services, coordinating all clinical pharmacy functions for the acute care areas as assigned to help achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams.
  • Completes patient profile reviews and participates in daily clinical care rounds, huddles, and meetings as an integrated member of a multidisciplinary team.
  • Reviews medication regimens for appropriateness, contraindications, and potential interactions
  • Collaborates with pharmacy supervisors, leadership, and staff to develop policies, procedures, and processes for more efficient processing within the pharmacy Enterprise and area of practice.
  • Works with multidisciplinary teams to develop, implement, evaluate, and update EHR order sets, Treatment and Supportive Plans to promote best practice care.
  • Develops an understanding of therapeutic clinical studies in their areas of expertise and manage patients' treatment based on study requirements and specifications.
  • Participates in Oncology research and contribute to cancer discoveries and the clinical trials being conducted by investigators and doctors at City of Hope.
  • Assists in designing, developing, conducting, and presenting the results of QI initiatives, Medication Use Evaluations (MUEs), and other projects related to patient outcomes.
  • Participates as a primary preceptor for pharmacy resident projects.
  • Participates in cost-saving and/or revenue-generating activities to contribute to the financial health of the department while ensuring safe quality care.
  • Performs other related duties as assigned or requested.

Qualifications

Your qualifications should include:

  • Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending
  • PGY1 and PGY2 residencies, or PGY1 plus two years of experience in an acute care hospital pharmacy, or Five years of experience in an acute care hospital pharmacy
  • Current CA Pharmacist License
  • American Heart Association BLS certification
