The Impact You Will Have

As part of our Medication Therapy Management team, the pharmacist will work closely with our members, their physicians, and care teams to provide comprehensive medication management services to optimize members' medication regimens. This patient care process includes collecting patient-specific information to assess all medication therapies a member is taking are appropriate, effective, safe, and able to be taken as intended. The pharmacist will identify medication related problems and create a plan to resolve them with the patient, caregiver to the patient, and/or the patient's prescriber. Pharmacist will be responsible for caring for members primarily telephonically or face to face documenting care, assisting in gathering data for program evaluation, and working to align the medication therapy management practice with our member experience strategy and with other services across Blue Cross.

This role is remote but will require current unrestricted Minnesota licensure to practice pharmacy.

Your Responsibilities:

  • Provide comprehensive medication management services to members primarily telephonically and face to face

  • Document comprehensive medication management services provided for the purposes of coordinating care with the member and their care team.

  • Coordinate care delivery and referral criteria with the BCBSMn Care Management team

  • Precept pharmacy learners at all levels, including students, interns, and residents

  • Actively participate in cross-functional care team meetings to improve clinical programs, enhance processes, and share clinical information

  • Provide comprehensive medication management services across all lines of business, including but not limited to Medicare, Medicaid, FEP and Commercial members depending on members needs

  • Actively participate in continuous quality improvement processes to enhance members' experience and optimize the medication management practice

  • Other duties as assigned to meet the needs of the members and communities served

Required Skills and Experiences:

  • High school diploma (or equivalency) and legal authorization to work in the U.S.

  • Doctorate of Pharmacy (PharmD) or equivalent degree.

  • Current unrestricted Minnesota licensure to practice pharmacy.

  • Have graduated from a college of pharmacy after May 1996 or passed an approved Accreditation Council of Pharmacy Education (ACPE) program that has both didactic and clinical elements.

  • 3+ years of related professional experience. All relevant experience including work, education, transferable skills, and military experience will be considered.

  • Strong clinical pharmacy knowledge.

  • Excellent interpersonal, consultative, and problem - solving skills.

  • Strong customer service skills.

  • Self-motivated, with demonstrated ability to work independently with minimal supervision.

  • Commitment to professionalism and a dedicated desire to advance patient care.

  • Excellent verbal and written communication skills.

  • Strong computer skills including Word, Outlook, PP and Excel.

Preferred Skills

  • Ambulatory care residency with an emphasis on providing MTM services

  • Board Certification in Ambulatory Care or Pharmacotherapy.


Compensation and Benefits:

Pay Range: $115,000.00 - $155,300.00 - $195,600.00 Annual

Pay is based on several factors which vary based on position, including skills, ability, and knowledge the selected individual is bringing to the specific job.
