Job Description
Job Summary: The Manager of Community Health Workers (CHWs) at Emcara Health is responsible for overseeing the CHW program, ensuring the delivery of high-quality, community-based care to our patients. This role involves managing a team of CHWs, coordinating with other healthcare professionals, and implementing strategies to improve patient outcomes and community health. The Manager, Community Health serves as lead to support our market CHWs across the enterprise for delivering services to the highest need members and assisting in closing care gaps. The Manager of CHW Community Health partners with VP of Care Delivery, Market Medical Directors and Directors of Care Delivery in strategies of leveraging CHWs in clinical initiatives to support the delivery.
Position Essential Functions: Manager, Community Health:
- Manages the entire CHW team, providing leadership and support to foster a collaborative and inclusive work environment
- Serves a limited caseload of clients in a similar manner as Community Health Workers, taking on more complex cases as needed.
- Partners with the Emcara Community Health Workers and Care Delivery Leaders to leverage CHWs in addressing the social (SDoH) identified needs, care gaps and health conditions that impact individual health outcomes and community health. This individual will support the integrity, advancement, and expansion of our home-based advanced primary care model through enhancement of current care solutions, as well as partner in continued innovation in this critical area of care delivery.
- Monitor and evaluate CHW program performance by using data and metrics to ensure continuous improvement.
- Provides an expert resource to our field-based CHWs, assisting in trouble-shooting and brainstorming options to assist with the coordination of care and services with community team members.
- Assists in the creation of training modules and continuing education curriculum for Community Health Workers.
- Partners to train new Community Health Workers and provides ongoing staff development instruction.
- Leads the work with other CHWs to update and maintain a source of community organizations and programs
- Works collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Works to reduce cultural and socio-economic barriers between clients and institutions.
- Assists with public presentations on program to interested audiences, including potential payers.
- Up to 25% travel time.
- Other duties as assigned.
- Education: Bachelor's degree in Public Health, Nursing, Social Work, or a related field. Master's degree preferred.
- Experience: Minimum of 5 years of experience in community health, public health, or a related field, with at least 2 years in a supervisory role.
- Skills: Strong leadership, communication, and organizational skills. Ability to work effectively with diverse populations and in a team environment.
- Knowledge: Understanding of social determinants of health and experience working with underserved communities.
Preferred Qualifications:
- Certification: Certification in community health or a related field.
- Experience: Experience in a value-based healthcare setting.
