Within 72 hours of a patient's admission, you will complete a thorough Psychosocial Assessment and conduct MMSEs as ordered by physicians, ensuring all documentation is clear, concise, and accurately reflects patient progress in the EPIC EHR. You will play a pivotal role in case coordinating the multidisciplinary treatment team, facilitating the development and execution of individualized treatment plans, and consulting with patients, families, and team members-including those involved in substance abuse services. Your expertise in navigating complex family dynamics, social/emotional needs, and dual diagnoses will inform comprehensive discharge plans. Acting as a liaison with the unit discharge planner and external community resources, you will help ensure safe, timely discharges that align with each patient's clinical readiness. Additionally, you will support patient and family understanding of treatment goals, empower decision-making, and ensure compliance with behavioral and medical recommendations.
