- Youth & Family Support - We help youth gain independence through social and living support programs while empowering families with the skills and resources they need to become resilient and successful.
- Foster Care & Adoption - We reunify and unite families while recruiting and providing support to foster parents and youth in foster care.
- Education & Community Trainings - We help students achieve academic success while giving educators the tools to create safe learning environments to improve their students' behaviors and offer innovative learning opportunities to build and improve knowledge in the community.
We are seeking a Manager of Treatment Foster Care (TFC) to serve as a leader to the team members who provide supports to designated TFC homes. The Manager has the ultimate responsibility of ensuring quality services provided to youth enrolled in the TFC service and the TFC homes that are in compliance with state contracts, funder requirements, and accreditation standards. The Cornerstones of Care TFC model is a collaborative approach to support specialized foster homes in caring for children with complex emotional and behavior needs. Our homes provide a stable, nurturing family-based environment alongside intensive therapeutic interventions and case management to address a child's specific challenges. The Manager plays a pivotal role in day-to-day operations of TFC program including, yet not limited to new home recruitment, provider/family training, client pre-authorizations and admissions, treatment planning, collaboration, safety, crisis management, support, resource support, and discharge/transition. WHAT YOU WILL DO: Your role will
- Provide leadership and support to TFC Specialists providing in-home support and coaching for Treatment Foster Care Homes, Relative Treatment Foster Care and Treatment Foster Care Transition Services to aid in the development and success of the placement as well as permanency for youth in the program.
- Assure services provided actively involve the child's family (biological parent(s), adoptive parent(s), foster parent(s), relative caretakers, legal guardians or adults who play a significant role) in the treatment and care of the child while in treatment foster care.
- Work collaboratively across program lines to ensure TFC program needs are met. This may include but is not limited to Outpatient, Resource Development, Foster Care Case Management, and Finance.
- Conduct and/or support the initial and on-going assessments/interview with placement participants.
As a member of the Youth and Family Support team, you will work with other team members and report to the Director of Youth & Family Support. WHAT YOU WILL BRING: Our ideal candidate will have 3-5 years of relevant experience and the following: REQUIREMENTS
- Education Level: Master's Degree
- Preferred Requirements: Professional licensure (e.g. LCSW, LPC)
- Years of Experience: 3 to 5 years in social work and/or child welfare
- At least 21 years of age and pass background check, physical and drug screening
- A valid driver's license, proof of current vehicle insurance and reliable transportation
