How You'll Help Transform Healthcare:
Assesses patient needs and assists with providing access and navigation to inpatient, community-based, and/or clinic-based services to those identified as being at-risk of serious health conditions or high utilization of the Emergency Department based on social determinants of health.
- Assists patients with accessing and navigating inpatient, community-based, and / or clinic-based services to those identified as being at-risk of serious health conditions or high utilization of the Emergency Department based on social determinants of health.
- Uses person-centered / trauma-informed modalities including motivational interviewing to engage and identify needs.
- Provides support, services, and ongoing coordination in addition to SMART goals and resource navigation.
- Accompanies patients to appointments (health / medical, educational, legal, employment-related, or benefits-related,) when needed to advocate and promote recovery and wellness.
- Facilitates communication and advocates for patients.
- Maintains documentation on the electronic medical record in hospital system.
What We Require:
Education: High School diploma or equivalent required.
Experience: Two years of general work experience. Experience in community outreach efforts as a volunteer or paid professional preferred. Experience in working with underserved populations or direct patient care/reception, registration and/or customer service/consumer relations preferred.
Licensure, certification, and/or registration: Community Health Worker certification preferred.
Life Support: AHA BLS-HCP must be obtained by end of supervised orientation and maintained
Other Minimum Qualifications: Community Health Worker training must be successfully completed upon hire and reviewed annually. Excellent interpersonal and communication skills. Must have reliable transportation, a valid driver's license, and proof of insurance. Computer word-processing and data entry skills required. Strong collaboration skills. Must work well with other staff, community, and family members. Extremely reliable, dependable, punctual, and honest. Bilingual in Spanish preferred. Experience using a smartphone, email and the internet is preferred.
