SUMMARY:Under general supervision and within Brown Health established policies and procedures the Sepsis Program Coordinator collaborates closely with physician leaders clinical leaders and staff to provide leadership strategic oversight and active management of the day-to-day clinical care delivery/operations of the sepsis program within Brown University Health hospitals. The Sepsis Coordinator is responsible for ensuring the provision of reliable high value evidence-based and coordinated care for sepsis patients in the ED and inpatient setting. The Sepsis Coordinator reports to the System Director of Clinical Abstraction Services in close partnership with the hospital/entity Directors of Quality as well as sepsis champions (physicians and providers) at each hospital. In addition to promoting SEP-1 compliance and leading improvement efforts related to sepsis care the Sepsis Program Coordinator will provide updates to the entity Quality Committees at each site on a periodic basis. This role will support the Director of Clinical Abstraction Services in the timely CMS reporting and submission for the SEP-1 Bundle compliance. The Sepsis Program Coordinator will facilitate evidence-based practice across all disciplines in accordance with current sepsis guidelines and will serve as a resource for staff and providers in developing plans of care. This role works closely with the ED teams interacts with the sepsis team activations (e.g. Code Sepsis) and facilitates improvement activities across the system. This includes creating or identifying performance metrics and data collection plans as needed ensuring continuous performance improvement. REQUIRED QUALIFICATIONS: Current Nursing licensure required. Bachelor*s Degree is required; master*s degree in public health Nursing Healthcare Administration Business or a similar field is preferred. 3+ years of relevant clinical experience preferably in clinical practice/acute care setting Self-starter independently seeking out and making use of available resources to develop new skills and subject matter expertise as needed Strong analytical and problem-solving skills Strong data analysis and summarization skills Strong leadership and negotiation skills Strong group facilitation skills Strong project management skills Excellent communication skills both presentation and written Proficiency in Microsoft Office Suite (Word Excel Access PowerPoint Visio) required Ability to work effectively in a complex and agile environment while remaining flexible and adaptable High-energy high-impact team-oriented professional that builds strong working relationships PRIMARY DUTIES AND RESPONSIBILITIES: Clinical Education and Advocacy: Maintains current knowledge of reporting guidelines and definitions for SEP-1 bundle compliance. Maintains competency in the management of the septic patient and in professional practice (e.g. conferences/workshops professional organizations etc.) Facilitates evidence-based practice across all disciplines in accordance with current sepsis guidelines. Serves as a role model consultant and clinical resource to medical nursing staff and other health care providers to promote health and reduce illness for sepsis patients. Performs real-time case surveillance to help identify opportunities to identify and treat sepsis in ED and inpatient settings including communicating with clinical teams and administrative leaders. Provides educational activities utilizing varied teaching/learning principles for clinical staff sepsis patients and their families. Rounds with identified healthcare personnel as necessary. Attends or remotely facilitates sepsis team activations (e.g. Code Sepsis). Serve as a professional practice and clinical expert at the bedside and in the classroom. Project Facilitation and Leadership: Assists in the development of policies procedures standards clinical pathways order sets and other materials. Builds and collaborates with report writing that will help the organization monitor quality of care metrics as needed. Performs chart reviews for areas of opportunities gaps in care and missing documentation that leads to non-compliance with the Sepsis bundle elements. Conducts validations for the missed opportunities noted in Sepsis Cases to identify trends in missed bundle elements. These missed opportunities are shared at each hospital with the designated Sepsis champions. Incorporates a feedback loop with the dedicated Sepsis champions so patient examples with missed opportunities and cases with positive bundle compliance can be shared. Works collaboratively with nursing leadership and quality departments to advance the development implementation and assessment of quality improvement activities. Work to identify remove and address barriers as they arise. Independently createsaudience-appropriatesummaries of relevant data sources identifies trends highlight actions taken in response to performance andanalyzethe effectiveness of those actions and contributing factors based on data. Perform other related duties as assigned. WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS Work is performed in a clinical setting. May also occur in a typical office setting requiring extensive periods of sitting with arms extended as well as walking and standing. SUPERVISORY RESPONSIBILITY None.
