Schedule, confirm, coordinate, and provide support to patients for in office and external appointments. 2. Contacts patients by phone to remind of upcoming appointment(s), to ask follow up care questions, and to provide special instructions if needed. Cancels and reschedules appointments as needed. 3. Must be able to communicate with patients, families, facility staff, and colleagues effectively and professionally 4. Display sensitivity to and experience working with different cultures. 5. Display comfort and interest working with members around issues of healthcare access and adherence. 6. Possess ability to multi-task and work under time pressures. 7. Demonstrate computer proficiency inclusive of ability to enter and access data/information. 8. Demonstrate ability to communicate both verbally and in writing with the members of the health care team. 9. Demonstrate ability to be a self-starter and work independently. 10. Completes all administrative functions associated with referral activities in a timely manner. 11. Works closely with Navigation team to assist patients with external resources as needed. 12. Ensures that all necessary participant information is documented in electronic medical records 13. Works collaboratively with the Bridgeport Hospital Primary Care Center Staff. 14. Performs other duties as needed. Qualifications EDUCATION High School Diploma or GED required. Associates Degree in health and/or human services or related field preferred or medical assistant certification preferred. EXPERIENCE At least 2 years experience in health care or in human services with a focus on community setting in health care or in human services or 1 year experience or related internship in health care or human service with a focus on community setting with an Associate's Degree. SPECIAL SKILLS Ability to work independently with support and supervision from Manager of Care Coordination and Director of Community Health and to also collaborate and communicate effectively with others, including patients and families, staff, and community health care providers.
