Description Maine Medical CenterProfessional - Nonclinical Summary: The Health Guide role is responsible for assisting patients as they obtain care at the community-based clinic serving people experiencing homelessness. This position is responsible for initial assessment and determination of appropriate level of service needed to address medically and socially challenging issues, health needs, and barriers for patients. The Health Guide will provide education around resources, programs, and assist patients with navigating the system and with applications as needed.
