This includes Clubhouses in Manhattan, the Bronx, Hollywood California, Harlem NY, and Care Management, Community Oriented Recovery and Empowerment Services, and a large housing program ranging from 24-hour supervised residences to scattered site supported apartments. As the HRSN Social Care Network Oversight Manager, you will play a pivotal role in the implementation of a groundbreaking Medicaid benefit-Health-Related Social Needs (HRSN)-and the formation of Social Care Networks (SCN) across New York State, ensuring that our work leads to systemic change and sustainable support for people living with serious mental illness. Full-time inhouse (Hell's Kitchen) Monday to Friday 9am to 5pm $70,000 to $75,000 salary
Requirements
Your Impact As the HRSN Social Care Network Oversight Manager, you will:
- Oversee all HRSN activities, ensuring efficient operations and effective service delivery to identify, screen, and navigate eligible members toward appropriate health-related social services that address unmet needs.
- Collaborate with other NYC-based clubhouses and Fountain House programs that serve Medicaid recipients.
- Create and manage tracking systems for screenings, eligibility assessments, referrals, and follow-ups within the SCN.
- Oversee and review staff documentation in SCN platforms for accuracy, communicate necessary corrections, and ensure resolution in collaboration with staff and supervisors.
Key Responsibilities: Program Strategy & Innovation
- Develop and implement systems to identify Medicaid status and ensure eligible members are screened, assessed, and navigated to internal and external HRSN services.
- Collect and analyze data to evaluate program impact and inform decision-making.
Member Engagement & Support
- Ensure SCN work is informed by social practice principles and aligns with the Clubhouse model, engaging members creatively and meaningfully.
- Coordinate with direct service staff across the organization to facilitate member engagement in HRSN services.
Operations & Compliance
- Ensure all HRSN screenings and navigations-whether in-person or remote-are completed promptly and meet established deadlines.
- Oversee and verify invoicing accuracy; collaborate with the Finance team to ensure timely reconciliation of payments.
- Demonstrate the ability to gather, submit and advise detailed business information related to all aspects of SCN's.
- Stay current on HRSN/SCN developments by attending conferences, trainings, webinars, and other professional learning opportunities.
Partnerships & Resource Development
- Educate internal teams about HRSN/SCN platform functions, procedures and protocols, opportunities, and system updates.
- Represent Fountain House and build partnerships with SCN's, HRSN Service Providers, and other referral sources.
Team Leadership
- Complete comprehensive training on SCN and HRSN protocols, including screening, navigation, service delivery, and documentation using platforms such as Unite Us and Find Help.
- Supervise a small team supporting SCN operations; ensure staff adherence to Fountain House and SCN policies and protocols.
- Conduct annual performance reviews and provide regular (weekly) supervision to direct reports.

What you bring
- Graduate degree in mental health, public health, or a human services-related field preferred; candidates with a bachelor's degree and equivalent experience will be considered.
- Minimum 2 years' experience in mental health or a related field required.
- Supervisory experience is preferred.
- Experience with the Clubhouse Model is a strong plus.
- Bilingual (Spanish/English) preferred.
