Per Diem Speech Therapist- Sebastion & Barefoot Bay

POSITION SUMMARY

Responsible to the Director of Professional Services, Clinical Manager to provide medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, Company Policies and professional Standards of Practice. Supervises personnel to deliver care and coordinates care delivery.

JOB RESPONSIBILITIES

• Participates in the development of patient plans of care.
• Makes visits to the patients for assessments and evaluations and communicates with the physician as needed. Implements the Plan of Care.
• Documents the care and submits electronic documentation in a timely manner.
