Job Details

As an Ambulatory Oncology Pharmacist, you will play a critical role in providing comprehensive pharmaceutical care to Breast Cancer and GYN malignancy patients undergoing outpatient treatment for cancer. You will work closely with oncologists, nurses, and other healthcare professionals to optimize medication therapy and ensure the safe and effective management of oncology-related medications to improve patient safety, enhance patient outcomes and prevent hospital admissions.

Job Description

Essential Functions:

  • Medication Therapy Management: Collaborate with oncologists to develop individualized treatment plans for patients, ensuring appropriate medication selection, dosing, and monitoring.

  • Patient Education: Educate patients and their caregivers on the proper use of oncology medications, including administration techniques, potential side effects, and strategies for managing adverse reactions.

  • Medication Review and Verification: Conduct thorough medication reviews to identify potential drug interactions, contraindications, and dosage adjustments, taking into account each patient's unique medical history and treatment regimen.

  • Adherence Support: Implement strategies to improve medication adherence among oncology patients, such as medication counseling, adherence monitoring, and refill reminders.

  • Clinical Consultation: Provide clinical consultation and support to healthcare providers regarding oncology-related medication therapy, including drug information inquiries, therapeutic interventions, and adverse drug event management.

  • Quality Improvement: Participate in quality improvement initiatives aimed at enhancing the safety, efficacy, and efficiency of oncology pharmacy services, such as medication reconciliation, protocol development, and outcome assessments.

  • Research and Education: Stay abreast of the latest advancements in oncology pharmacy practice through continuing education, literature review, and participation in research projects, contributing to the advancement of evidence-based oncology care.

Minimum Education and Experience:

Education:

  • Doctor of Pharmacy required

  • PGY1 Residency from an ASHP accredited program - Pharmacy Practice OR equivalent experience (highly preferred)

  • PGY2 Residency from an ASHP accredited program - Oncology OR Board Certification in Oncology (BCOP) (highly preferred)

Experience:

  • Minimum 2 years Pharmacist Experience (preferred)

  • Ambulatory Oncology Experience (preferred)
