$63,999.94-$79,999.92 The Rose F. Kennedy Center's Quality Improvement (QI) Coordinator is responsible for implementing and coordinating the Quality Management program of the Developmental Medicine Pediatric Division at Montefiore Medical Center. The QI Coordinator works closely with the Senior leadership to plan, organize, direct, and lead the personnel and work processes of the quality improvement program according to the standards of our Article 16 License with OPWDD. The QI Coordinator uses data and tracking systems to assess, monitor and report program performance and determine ongoing improvement needs. The QI Coordinator also uses this data to report information pertaining to standards compliance, clinical performance, and patient experience to internal and external entities. The QI Coordinator chairs the quality improvement committee and assures execution of quality management initiatives throughout the Division. The QI Coordinator is responsible for ensuring compliance with Article 16 regulations that require dissemination, coordination of clinical services with non-Montefiore treatment providers. Responsible for ensuring no duplication of services between CERC/ DOE/ OMH and OPWDD treatment settings occurs. Documents coordination efforts into the patients EPIC for over 500 patients active on therapy with treatment plans Participates in treatment/program planning and maintains appropriate records, statistical reports, etc. so as to ensure appropriate treatment activity as well as maintain program efficacy.
Has extensive knowledge in Article 16 regulations and care coordination. Has deep understanding of community partners and is able to build transparent partnerships around patient activity. Initiates and problem solve utilizing available resources within level of responsibility. Requires knowledge of interdisciplinary treatment settings.
Training required- OPWDD CCO trained, Medisked trained
Minimum 3 years of work-related experience
