City/State: Bronx, New YorkGrant Funded: NoDepartment: NoMgrWork Shift: DayWork Days: MON-FRIScheduled Hours: 8 AM-4:30 PMScheduled Daily Hours: 7.5 HOURSPay Range: $127,720.01-$159,650.01 The Clinical Transplant Coordinator, in collaboration with physicians and other healthcare professionals, is responsible for managing the clinical care of patients and families throughout the liver transplantation process. This role also involves close collaboration with transplant surgery, hepatology, and referring providers to optimize care coordination for patients with malignancies such as colorectal cancer and hepatocellular carcinoma (HCC). Key responsibilities include active participation in tumor boards, coordination of multidisciplinary treatment pathways, and contribution to the development of protocols for managing complex medical conditions associated with liver failure.The coordinator applies broad clinical expertise while serving as a clinician, educator, collaborator, and patient advocate. Functioning in a semi-autonomous, self-directed capacity, the coordinator provides both direct and indirect patient care in accordance with professional practice standards and evidence-based clinical guidelines. REQUIREMENTS: Bachelors degree in Nursing required Current licensure and registration (or valid permit) as a registered professional nurse with the New York State A minimum of two years of clinical experience in a highly specialized complex care program. Spanish speaking preferred Experience in transplant patient care role preferred. Effective English communication skills, both written and oral Ability to use data entry and retrieval system.
