Description

Our Family Preservation (FP) program provides home based casework services for multi-problem families. The home-based casework is also available for pre-adoption and post adoption services for adoptive families at risk or in crisis. Benchmark Human Services is hiring an In-Home Case Manager to provide direct service to youth and families by addressing the short-term and long-term needs of the family by working to build upon the strengths of the family.

The Case Manager will travel for in-home visits in regions 11 & 12 of Indiana (Timpton, Madison, Hampton, Hancock, Henry, Wayne, Rush, Fayette, Union, & Franklin Counties).

General Responsibilities:

The In-Home Case Manager will provide direct service to youth and families by addressing the short-term and long-term needs of the family unit - including pre and post adoption. The Case Manager must be willing to travel and have a flexible schedule. Services will be provided in a wide variety of settings, ranging from in-home to various community settings including but not limited to schools, doctors' offices, professional office settings, etc. This position will provide whole-person and family centered care coordination across all domains of human services, especially medical, behavioral, legal and social services.

Specific Job Responsibilities:

  • Develop and implement individualized case management plan focusing on the identified needs from the clinician's report.
  • Provide required weekly face-to-face visits with each child in the home. This minimum will be completed regardless of any case management or therapeutic activities during the week.
  • Assist with developing a budget with family unit to ensure they can meet concrete needs of the household.
  • Ensure proper referrals and linkages are made to required services including, but not limited to, mental health services, counseling, tutoring, spiritual, medical, financial, vocational, substance abuse, educational, etc.
  • Assist family unit with ensuring essential bills are paid and linkage to funding assistance programs are secured.
  • Ensure that safety standards continue to be met and that family protective measures are intact and utilized appropriately.
  • Provide supervised visitation as outlined by DCS; and document supervised visits as required by Service Standard.
  • Provide parent education utilizing evidence-based programs to improve family communication, social interactions, conflict resolution.
  • Participate in on-call rotation to ensure safety needs of family unit are addressed 24/7.
  • Complete all required daily, weekly, and monthly paperwork within guidelines established by the Department of Child Services and Benchmark.
  • Attend all required Benchmark, DCS, and other required external meetings.
  • Other duties as assigned.
  • Comply with all standards to ensure the health and safety of all individuals.
  • Report to work as scheduled.

Qualifications:

Minimum:

  • High School Diploma, GED or associate's degree with at least 4 years of full-time employment providing direct case work services to children and families.
  • OR must possess a bachelor's degree from an accredited college or university in one of the following disciplines: psychology, sociology, mental health, social work, counseling, or any other discipline within human services health field.
  • Prior experience working with youth and families.
  • Valid driver's license and auto insurance
  • Must be computer literate.

Preferred:

  • Master's degree from an accredited college or university in the one of the following disciplines: psychology, social work, sociology, marriage and family counseling.
  • Experience working as a case manager with youth and families within DCS or Probation systems of care.
