Wage Range: $68,640 - $84,024.91 per year
Salary is based on geographic location and relevant experience

Primary Responsibilities

As a master's level case manager, you will:

• Provide in-home clinical case management to seniors living throughout San Diego County

• Offer treatment planning & direct support to help members pursue their recovery goals and housing stability

• Complete comprehensive biopsychosocial assessments, risk assessments, clinical documentation and safety plans in accordance with CalAims, county, and Telecare standards

• Participate with an enthusiastic team of case managers to engage in case consultations and coordinate care

• Provide psychosocial rehabilitation and therapeutic services to members and their families

• Collaborate with community resources including the Conservator's Office, Public Guardian, Probation Department, and inpatient or housing providers

Qualifications - What You'll Bring to the Table

• Must be registered with the California Board of Behavioral Sciences (ASW, AMFT, or APCC)

• Master's Degree in Social Work, Marriage & Family Therapy, Counseling, or related field

• Minimum of one (1) year experience working with individuals with serious mental illness

• Valid California Driver's License and current auto insurance with your name listed as a driver

• Willingness to use your personal vehicle to transport clients as needed (mileage reimbursed)

• Ability to work collaboratively as part of a multidisciplinary team

About the AgeWise Program

AgeWise is a community-based program that supports San Diego County residents on MediCal who are aged 60 and older and living with a serious mental illness

• Total Members Served: Approximately 350

• Program Type: Strength-Based Case Management (SBCM)

• Referral Sources: All referrals are made through Optum's Single Point of Access (SPOA) by community providers, family members, or self-referrals

• Hours of Operation: Monday - Friday, 8:00 AM - 5:00 PM (Crisis services available 24/7)
