Maternal Child Care Coordinator, Remote in WA

As a Maternal/Child Care Coordinator, you will act in a liaison role with Medicaid members and their families to ensure appropriate care is accessed as well as to provide home and social assessments and member and family education. Care Coordinators will integrate care for enrollees across multiple delivery systems working to reduce gaps in services and increase coordination of all service providers including medical, behavioral health, long-term services and supports, and other social services. Interest in learning to work with medically complex patients is important for this role.

If you reside in the state of WA, preferably in the Greater Seattle area, you will have the flexibility to work remotely* as you take on some tough challenges.

Expect to spend up to 25% of your time in the field visiting our members and families in their homes or in long-term care facilities. You'll need to be flexible, adaptable and, above all, patient in all types of situations.

Primary Responsibilities:



  • Create a positive experience and relationship with the member and their families
  • Help member and/or their family set goals and develop a care plan to achieve those goals
  • Proactively engage the member and/or their family to manage their own health and healthcare
  • Support the member and/or the member's family to improve the member's wellbeing by staying out of the hospital, and attend regular visits to their primary physician
  • As needed, help the member engage with mental health and substance use treatment
  • Provide member and/or their family education on community resources and benefits
  • Conduct post discharge activities from hospital facility and support connection to social services
  • Partner and collaborate with internal care team, providers, and community resources/partners to implement care plan
  • Provide education, health promotion, and coaching to support member and their family's management of care needs to promote health
  • Advocate for members and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team
  • Knowledge and continued learning of community cultures and values
  • Conduct member assessments

Required Qualifications:



  • Meet one of the following

    • LPN
    • 2+ years of relevant experience, including 1 year of clinical experience
    • CHW
    • Certified Medical Assistant
    • Certified Nursing Assistant


  • 2+ years of experience working with the pediatric/ youth population; preferably in NICU and/ or Pediatric clinical experience
  • 1+ years of experience with MS Office, including Word, Excel, Outlook, and TEAMs
  • Driver's license and access to reliable transportation and the ability to travel up to 25% within Washington State to meet with members and providers. *Travel time can change depending on the needs of the business
  • Reside in the state of Washington



Preferred Qualifications:



  • 1+ years of community case management experience coordinating care for individuals with complex needs
  • 1+ years of NICU and/or Pediatric clinical experience
  • Background in Managed Care
  • Experience working in team-based care
  • Experience working with the pediatric and/or NICU population
  • Reside in the Greater Seattle, WA area
  • Experience with Maternal Health
  • Bilingual: English and Spanish


Physical Requirements:



  • Ability to transition from office to field locations multiple times per day
  • Ability to navigate multiple locations/terrains to visit employees, members and/or providers
  • Ability to transport equipment to and from field locations needed for visits (ex. laptop, etc.)
  • Ability to remain stationary for long periods of time to complete computer or tablet work duties

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $23.41 to $41.83 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
