The Sr. Patient Services Coordinator (PSC) plays a dual role, offering both compassionate patient engagement and meticulous data management in support of the transplant community. This hybrid position combines the interpersonal outreach of a PSC with the data quality responsibilities of a Quality/Data Coordinator. You'll be a critical link between patients and care teams-helping patients stay engaged and supported-while also ensuring the accuracy, completeness, and timeliness of transplant-related data systems.

We'll equip you with the training you need-bring your curiosity, compassion, and attention to detail.

Key Responsibilities:

  • Patient Engagement & Clinical Support
    • Call patients regularly to complete wellness and engagement check-ins
    • Provide patients with basic educational guidance and encouragement
    • Call medical providers and transplant teams to gather and update records
    • Assist with surveys or communications
    • Document all interactions professionally and accurately in appropriate systems
    • Follow written protocols and scripts for consistent task completion
    • Attend virtual team meetings and on-site trainings as scheduled
  • Data Quality & Administrative Support
    • Maintain waitlist databases and conduct routine quality assurance reviews
    • Enter and update data in electronic medical record platforms
    • Create and maintain Excel tracking spreadsheets (e.g., for captured changes)
    • Support monthly/quarterly reporting on transplant activities and metrics using excel spreadsheets.
    • Backup support for other Patient Service Coordinators and supervisors as needed
    • Develop and maintain documentation on how to get access to centers and process protocols.
    • Monitor and help improve key data quality metrics: Accuracy, Completeness, Consistency, Timeliness, Validity, and Uniqueness

Qualifications:

  • 3 years administrative or clinical support in hospital/healthcare/clinical settings required
  • High school diploma required, college preferred
  • Bilingual (English/Spanish) strongly preferred
  • Strong written and verbal communication skills
  • Organized, self-motivated, and able to work independently
  • High attention to detail; skilled in data entry, validation, and reporting
  • Proficient in Microsoft Office Suite (Excel, Word), OneDrive, and general computer literacy
  • Able to learn new software and databases independently
  • Comfortable speaking with patients and medical professionals
  • Quiet, private home environment for handling PH
  • Flexibility to support different time zones and shifting workloads
  • Willingness to travel periodically to San Antonio for training

Preferred Skills:

  • Intermediate/Advanced Excel skills
  • Familiarity with data visualization tools and dashboards
  • Experience with QA reporting and continuous improvement practices
  • Strong time management and problem-solving abilities
  • Leadership/mentorship mentality

Remote: US only roles

The anticipated base salary range in the United States is $21 to $30/hr. The final salary offered to a successful candidate will be dependent on several factors that may include but are not limited to the type and length of experience within the job, type and length of experience within the industry, education, etc. Base pay is one part of the Total Package that is provided to compensate and recognize employees for their work, and this role may be eligible for additional discretionary bonuses/incentives, and restricted stock units. CareDx is a multi-state employer, and this salary range may not reflect positions that work in other states.
