Summary:

Community Health Workers (CHW) are both respected members of the communities they serve and key members ofMH's Community Health/Food Is Medicine team. They positively impact the health of our patient populations through addressing the social, medical and community problems that may contribute to poor health. The CHW will assist patients to access health resources/services, support multi-lingual volunteers and will assist in the coordination of culturally appropriate care and job-seeking services for patrons and volunteers at the MH Food Pantry Portland.

Language- must be bi-lingual in English and French

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: High school diploma or GED preferred.
2. License/Certifications: If driving required for role, Valid Driver's license, clean driving record, and personal vehicle for use required.
3. Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
4. Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community.
5. Additional Skills/Requirements Preferred: Basic computer skills preferred.
