Palliative Care Physician, MGB - Salem Hospital Provides clinical leadership and expertise within the palliative care services team to improve patient care throughout the hospital for patients with complex illness and patients approaching end-of-life illnesses. Builds effective relationships with referring physicians to ensure excellent team followup regarding patient consults and to build an improved culture of collaboration among specialists, primary care physicians, nurses, and other staff to meet the needs of palliative care patients. Provide consultation in palliative care, symptom management and supportive care for the patient to meet the general medical needs of the patient to the extent that these are not met by the attending physician. Supervise activities of palliative care Nurse Practitioner and other clinical and nonclinical care staff associated with the palliative care program. Facilitate clarification of patient and family goals of care. Facilitate access to appropriate support models of care. Provide second opinion regarding need for hospice care with official certification provided by the Hospice Medical Director. Education of staff and co-workers. Consult with the attending physician and the Interdisciplinary Team in establishing a written plan of care at intervals specified in the plan. Assist in the development of standards of care.
