Partners with patient and caregiver to promote wellness and adjust lifestyle or health concerns. Supports patient and caregiver in setting short- and long-term personalized health goals promoting a patient-centered vision of health and well-being. Inspires patient and caregiver to achieve their optimal well-being through healthier lifestyle choices.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Bachelor's degree in health education, athletic training, exercise physiology, nutrition, counseling or other health related field. 2. National Board-Certified Health and Wellness Coach by the National Board for Health and Wellness Coaching (NBHWC) within 6 months of hire. MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Master's degree in health education, athletic training, exercise physiology, nutrition, counseling or other health related field. 2. Previous experience working in a healthcare or health related setting. CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. Provides necessary coaching to support individualized behavior change. 2. Utilizes the coaching model process to help patients clarify their vision and values, assess readiness to change, determine focus and set goals, prepare for action, and maintain behavior change. 3. Actively listens, reflects and motivates patient in establishing SMART goals to optimize outcomes. 4. Establishes and maintains patient-centered relationships that recognizes the patient is the expert in navigating their own life and provides the structure to serve as a facilitative partner. 5. Contacts and performs initial interview with patients who are eligible for the health coaching program. 6. Identifies and provides appropriate resources to patient and or the caregiver where needed. 7. Provides education to patient and caregiver(s) on health and well-being, distributing health education materials as needed or requested. 8. Coordinates and collaborates with patient's healthcare provider as appropriate to provide a well-rounded experience and optimize outcomes. 9. Supports operational aspects of WVU Medicine / United Hospital Center to promote patient satisfaction. Must have reading and comprehension ability. 6. Must have manual dexterity to operate keyboards, fax machines, telephones, and other office equipment.
