Pay From: $27 per hour

MUST:

Experienced Licensed Practical Nurse - Urgent Care

2+ years of professional experience as a Licensed Practical Nurse - Urgent Care

Experience working with un-housed populations, substance use disorder and un(der)treated mental illness

Recent acute care experience in medical Emergency, Urgent care, or other inpatient units

Experience in/passion for public health and community nursing

Ability to perform well in high-pressure or stressful situations

Excellent communication and interpersonal skills

Excellent computer skills and proficiency with the Microsoft Office product suite

Independent decision maker and able to lead and manage people in a motivational and constructive way, individually and in a group setting.

Experience with the following EMRs is a plus: eCW, Epic

Current DC Licensed Practical Nurse license

Current BLS/CPR certification


DUTIES:

Provide comprehensive nursing care, which may include (but is not limited to) wound care, assistance with medication management, pain control, assessment of external body conditions, monitoring of vitals and recovery progress, and other health support as indicated by the care plan and referring entity.

Gather information for a clinical care plan, including physical and psychological history and assessment; list of current medications, dosages, and allergies; vital signs; other pertinent information, such as laboratory or other test results.

Provide medication management in accordance with state regulations and nursing best practices, to include storing, administering, reconciling, monitoring, and requesting refills as indicated by the care plan.

Provide clinical care within scope and license, as indicated by care plans.

Safely administer medications as ordered to clients unable to manage their own medication, typically a combination of oral meds, inhalers, short and long-acting insulin, and patches.

Educate clients on medication administration and management, overall health management and condition monitoring.

Supervise the CNA, ensuring that they are fulfilling all their responsibilities over the course of the twelve-hour shift (vital signs, ADL assistance, documentation, etc).

Monitor closely for changes in health status and respond accordingly.

Complete wound care and other nursing interventions as ordered by the Medical Respite provider and in accordance with the plan of care.

Communicate and develop plan of care with the medical respite provider.

Complete daily or frequent nursing assessment and/or medical exam as indicated by care plan.

Identify and implement strategies to support independence in health management.

Document all clinical care provided during medical respite stay.
