Job Description: ESSENTIAL JOB FUNCTIONS
- Make appointments and/or referrals for PCP or Medical Specialty.
- Maintain the Integrated Care Coordinators' schedule, including appointment reminders, scheduling appointments, and rescheduling following a cancellation or no-show.
- Prepares and mails care contacts to clients determined to be at a risk of suicide as directed by the Integrated Care Coordinator.
- Assists clients in navigating the healthcare environment to ensure appropriate access to care.
- Assists with coordination/access to services that address social determinants of healthcare outcomes:
- Assist with options for transportation to healthcare appointments
- Assist the client in obtaining housing resources
- Assists with access to other community resources, e.g., public assistance and financial benefits
- Develops a knowledge base of other healthcare providers in the service area and collaborates, as needed
- Documents tracking for appointments, medication, and other healthcare needs for assigned clients, as needed

- High School diploma or GED
- Preferred, but not required training as a Medical Assistant
EXPERIENCE
- One year of experience working in a hospital, physician's office, or a behavioral care clinic
- Experience in a community setting or an integrated care environment.
PHYSICAL DEMANDS
- Position may require lifting of 20 pounds maximum, with lifting or carrying of items weighing up to 10 pounds, or require significant periods of walking or standing
- Normal tasks may involve exposure to blood or bodily fluids
- Reliable transportation for travel between clinic locations if needed.
